    i would guess that the round either 'poped' out of the magazine befor it slid under the extractor (weak mag. lips?) or the rim of the top round was in back of the next round in the magazine and the slide set the primer off when it tried to strip the top round into the chamber.

    Might this be a situation attributable to "limp-wristing?"If the lady had never fired a weapon before & was taken aback by the recoil we're all used to, could this have changed the timing & inertiaof the slide on it's return to battery?What age is the recoil spring on this particular firearm? Does it compare with similar springs in like-model pistols in the armory?

    Had this happen to me in my own MKII, too much lube resulted in "caking" in the chamber, the round couldn't seat all the way in, and the extractor set off the primer.
