Need Help With C&R Application.

Just received my C&R application and need some help. First on form ATF 5330.20 line one it ask for name which application was filed(that would be me), then it ask name of person CERTIFYING to citizenship, would this be my name to?
Second question is where do I find the name and address of my local Chief Law Enforcement officer?

For the second question first, the local sheriff or chief of police would be who your looking for. Call the local sheriff or police station. for the first question I don.t remember. hope this helps

Also when you get your license send a signed photo copy to Century Arms, Southern Ohio Gun, and Interordnance.

DO NOT SIGN THE ORIGINAL! Make copies and sign the copies in any color ink except black.
