    Could be a Firing Pin/Spring Problem...Have you consulted a reputable Gunsmith or contacted the Factory who made the gun...?

    colt upper/essex lower. upper all series 70 colt. essex lower ???
    in the past occasionaly hammer would follow slide back into battery.yesterday multi fire. (45 auto.)

    Hammer and sear would be the first thing to look into, In addition your sear spring could just be worn out ,or need to be tightened ,maybe didn't seat well on the last tear down
    ,this can also be caused by a miss fitted thumb safety {yes it works in relation it plays a bigger role than just a safety}


    Have you had any of the above worked on ?,if not I would look into the sear spring ,and maybe bend it a bit to tighten it up ,then check the gun {unloaded of course} see if you can push the hammer down when fully cocked ,then rack the slide with gun pointed down ,pull on rear of slide real fast a few times ,see if the hammer is following the slide ,also try ,holding the gun ,rack the slide ,pull the trigger ,with out taking your finger off the trigger ,rack the slide again ,see if the hammer falls
