B-Square Scope Mount ID

I'm disposing of some estate items and need to ID this scope base.
What gun will this fit and what's that fitting on the lower forward (or is that the rear)part of the base? There are no identifying numbers on the base. It's a 5.25" Weaver-type base. See it at:

  Its a B-square Model #42406, and will fit all K, L or N frame S&W's with adjustable sights and 5" or longer barrels. The forward end is the one with the "ears". The other thingy fits into the rear sight notch on the revolver's frame. There should also be a small diameter screw which goes through the hole in the middle of the base and into the rear sight retaining screw hole in the frame.
