Permit question (CCW)

I'm not the smartest person so I will ask(as I cant find examples)
What I need is to find examples of "Letters of Cherecter referance"
I need 2 in the state of utah but do not know how to word nor do the people writing the letters. Does anyone have their letter or know how to write them that can help me out? or maby a place online that I can look at the layout and wording?? I'm not asking for a letter from people I dont know just how to write it and what needs to be listed and stuff like that. any help would be great!!
