Class III repairs - what paperwork is necessary?

I asked this question in another thread on the General Discussion side of the forum but not received an answer. My question is:

What paperwork is necessary to send a class III gun back to the manufacturer for repairs? Is it necessary to "transfer" the gun back to them, then have them "transfer" the gun back to me? Or can a class III gun be sent to the manufacturer and then be returned to the owner without paperwork, like a rifle, pistol and shotgun?

  Easy one!
  Take one a copy of your form4 write on the copy "For repairs to XXXXX"
  XXXXX= the repair facility, with the date and sign it....
  Send the copy of the form4 with the weapon.
  Its a done deal.
