lefever shotgun value help

i have a lefever double barrel shotgun and looking for the value and age.
serial # is 36004 h
gun is in pretty good condition for its age.

not sure of the model
it is a 12 gauge
barrel length from memory about 30 in.
it has damascus type barrels.
i think it is "h" grade that is the letter after the "36004"
any help would be appreciated
