Turned out to just be a powder buildup.

A lot of times the spring is broken or missing.

Ok, after some more testing, I have realized that what is happening is the part of the loaded chamber indicator that is in the front of the slide (to test for the bullets) is drifting up into the slot above it. It appears to be hitting the top of the barrel and always sticking out. When I take the slide off, and make sure the pin is down at the bottom, the loaded chamber indicator shows the chamber as being empty. But when I fire the last round in the magazine and close the slide manually, it still shows the chamber as being loaded. Is this a common problem, or is that just the way the gun was designed? It doesn't appear that there is anything wrong with the spring because the pin can move back and forth easily, but it drifts up into the slot above it (I assume that is why the slot was there?)
