    It mounts directly to the T/C barrel

    They don't mount to weaver bases. Instead bases were made specifically for it to fit a variety of handguns and muzzleloaders. They often can be purchased from on-line auction sites or occasionally found in gun stores that have been in business since the late 60's or early seventies.

    Probably - I think it was called "The Phantom" or something close. The underside has what looks like an integral Weaver rail, it's supposed to slide on to a clamp type mount that in turn attaches to a base on the barrel.
