  Not sure? Does it have any numbers or a name brand on it? Failing identification, Nordic Components makes a nice one that has Piccattiney rails left and right instead of an undersling such as you pictured.

  Don't know about that one.
  I like the Elzetta I have on my 870.
  Lite Mount Technologies makes a good one.
  Advanced Technology makes one also.

  Trick question. .
  
  What you have there are TWO SEPARATE mounts.
  
  Its a Fenix TK11 flashlight, held by a VTAC (Vikings Tactical) mount, onto a Laserlyte shotgun rail.
  
  So contact Vikings Tactical and Laserlyte if you want that EXACT setup.
  
  But before you do that, if I were you, I'd read the user review of the products that goes with the image you lifted:

  NTOA has Tested and recommended the Elzetta shotgun light system. All that and made in the USA as well... Might be worth a look.
