beretta pintail

I have a Beretta Pintail semi-auto 12ga.when I shoot 3 rapid shoots reload shoot another 1 or 2 shoots the port stays open and I have to release it,it doesn't always do this but it has done it probably 10 times last hunting season,can you tell me what you think? Thanks

  Does anyone know of a mag extension for a beretta pintail (es100 or vittoria)
