Wichester model 12 problems

i have a model 12 when fired the slide come back empty gets kicked but it will not pick another one up to chamber? any ideas?

Look at part #28 cartridge cutoff. Could be bent, broken or plugged from operating by crud.

Check the magazine tube for crud or dent(s).. also check the threaded area of tube for roundness.
If dented it will keep the follower from pushing a new round onto the lifter....

I should have said inside the magazine tube....
