Could be a broken firing pin. Check the bolt face for a burr or peened out area around the firing pin. Examine the rear of the chamber, the extractor and ejector areas. Could be weak or broken trigger/sear spring, crud build up not letting full engagement of trigger and sear. Take it apart and clean and lube it. Inspect all parts for wear or a bubba trigger job.

In a slam fire, does the hammer come forward?
If not soak the bolt assembly in Kroil , kerosine or ATF.
The firing pin should move freely and not protrude beyond the rear of the bolt face.
