Shotgun Recognition

Hey there folks. I'm trying to figure out what this is. I typed in 'c-lect-choke' in google and Mossberg 500 kept popping up. Is it safe to say that's what this is or is it something else? The only print I see on it anywhere is what I put in picture #4. I can take any other pictures that you'd need to help identify it! Any help would be greatly appreciated. Thanks!!!

  Mossberg, all right. C Lect Choke is the Mossberg trademark for their Polychoke copy.
  
  I don't know for sure why the gun is not marked Mossberg, perhaps it is a chain store contract gun.

  look on the bottom of the receiver just north of the loading port, should be model I.D. there.

  It's a Mossberg 500, but it may be marked as a "Revelation", "New Haven", or other brand
