    Tough to diagnose from here, but I'll take a WAG.

    Light loads can often do this in any wheel gun, full loads rarely. I think that there is perhaps a burr at the end of each chamber that is keeping the cartridge forward and not allowing it to back up and reseat the primer. The easy tool to check for the burrs is a Split Ball gauge.

    So are you using 2 round, 3 round Half Moon or 6 round Full Moon clips? Maybe something is off enough to bind the cartridge producing the same effect as a burr.

    I would try some 45 Auto Rim and see what they do.
    I would also check to make sure the crane is straight. A bent/crooked one can cause binding.

    Never Dick Tracy your pistol (rocking your wrist to get the cylinder to slam closed), always push it closed.

    added The 1955 that I shot for a while would function fine without the clips, extraction was a pull them out one at a time proposition. You are taper crimping your ammo? A roll crimp should be fine with clips.

    The 25-5 was in 45 Colt. My 8 3/8 did snake loads very nicely. Really thin in the bolt notch, don't try and hot rod one.

    added2 So for less than $25 I got this set to replace the Starrett set that turned up missing. Nothing to write home about, I slicked them up some with a stone and emery paper.

    Primers backing out is an ammo problem, not a gun problem.

    if this is a .45 acp (as opposed to .45 colt) you have to use the clips for proper headspace (or .45 auto rim cases).

    I use full moon clips and the ammo works fine in other 625's.

    I'm wondering about the cylinder being rough and not letting the cases recoil enough into the frame to push the primers back in...

    Moon clips may be too thin giving excessive headspace for rimless 45ACP on that particular 25.
    Compare measured distances between recoil plates and cylinder rears on your S&W 25s.
    If the subject gun shows a greater distance, you may need thicker moon clips or a new, fitted cylinder.
    Backed out primers can indicate too light loads in a handgun or excessive headspace in a rifle.
