I would not want to be the mechanic or person who put that wheel on.
A blowout is unpredictable but a tire coming off the truck, I would think happened because of an incorrect installation of the tire.

That tire really punished that vehicle. Tossed it into the air like a leaf and came back to kick it in the rear at the finish.

I was reading about this at lunch today. Supposedly the Kia Soul, is one of the safest vehicles on the market. Very strong exoskeleton. Rates higher than both of Elon Musk's Tesla's. (What Elon touts as "the Safest Vehicles in the World.") 🤔
The only reason the Kia Soul did not take "Top Honors," in the Safety category, was not having the best headlights that are available on the market today.
